Coordination of Supports
Our Enhanced Support service involves a multi-disciplinary team of professionals who work together to provide a coordinated model of support.
This means you will have access to a range of local professionals all in one place - including psychologists, speech pathologists, occupational therapists, social workers, behaviour support practitioners, and early childhood educators.
To help you achieve your goals, we also work with services, health professionals, and other informal supports from your wider network.
Specialist Support Coordination (funded under the NDIS)
We can also offer Specialist Support Coordination for people who need higher levels of support to manage risks, address any barriers, and to coordinate their services.
This service helps the individual build their own skills and resilience, and may include the development of a plan to ensure consistent support is provided.
How can I access Enhanced Supports?
We offer a variety of ways to access our services.
We are a registered provider of supports under the NDIS and can provide services approved and funded in your NDIS Plan. The most common NDIS categories of support for Enhanced Supports include:
- Improved Daily Living
- Improved Relationships
- Support Coordination
Medicare: Chronic Disease Management Plan
We are registered providers of Medicare and are able to offer a subsidy to the cost of services we provide. Services which may be covered under this plan include:
- Occupational Therapy
- Speech Pathology
We also welcome individuals, professionals, and organisations to contact us for private fees.
